Advanced Care Planning

Advance Care Planning is important for all adults, not just seniors or people who are seriously ill. While 82% of Californians say it is important to have end-of-life wishes in writing, only 23% have done so. Studies show that 50% of us will be unable to speak for ourselves during a medical crisis at some point in our lives.

Speak up now – so someone can speak for you in the future.

Advance Care Planning is a thoughtful process you carry out before a health crisis occurs or you become unable to speak for yourself. It involves exploring, communicating about, and documenting your wishes for the type of future medical care and treatment you would want. Advance Care Planning leads to completing an Advance Health Care Directive.

It’s often difficult to talk about the medical care you want should you become seriously ill or have a health crisis. My Care, My Plan offers workshops and resources to help you.

Advance Care Planning gives you peace of mind for the future. It also helps to ease the burden on your loved ones and health care agent (if you name someone to speak for you). Advance Care Planning is the most compassionate gift you can give to the important people in your life. Your decision makers won’t have to guess what your wishes are because they’ll already know.
