Tips for a good transition home from hospital or nursing facility:

  • Ensure that discharge paperwork has been explained and available to and understood by client
  • Ensure that there is sufficient prescription supply until you can be seen by Primary Care Physician (PCP) if refills are needed
  • Schedule and keep discharge follow-up appt with PCP
  • Ensure that medications have been reconciled (process of making sure that the medications you have in the home match up correctly with what has been ordered by your doctors)
  • Ask for print-out of reconciled medication list to keep in wallet/purse
  • Ensure that necessary support system is in place (referrals to Home Health, caregiver, Durable Medical Equipment, etc.)
  • Become familiar with what signs and symptoms to look out for that would warrant medical attention
  • Become familiar with who to call for questions or to report concerns
  • Become familiar with where to go for care (PCP appointment, urgent care, ER)
  • Update personal health record as health status or medications change
